Carcinogen vs.Host in Stomach Cancer
Suk Dongsoo
Abstract
Today, many people of advanced countries and developing countries alike are dying by cancer, and fortunately, we are gradually identifying their carcinogenic agents.
There are two solid necessary items, carcinogen and host factors, to produce cancer in experimental animals as well as in hurrianbeings. As far as experimental model is concerned, it is generally true that stronger the carcinogenic agents, shorty the latent period of the cancer, and more rapidly growing in the nature of the cancer. Likewise, stronger the hereditary oticoger is tendency of the host, shorter the latent period and more rapidly growing the cancer. Therefore, it is very reason-able to expect_ the similar phenomena on human cancers.
Tso and his associates reported that they found 38 cases of gastric cancer in patients of 35 years old and younger out of 1711 cases of stomach cacer during the period of 1948 to 1983." And they noticed that males and females are affected equally in this younger age group. Histologically, diffuse infiltrating type is more common, and they are predominantly lesions of advanced stages. For there was only one patient with 5year survival in this particular group. Holburt and Freeman examined stomach cancer among resident of Los Angeles.a Eventhough they found only 18 patients
Department of Pathology, College of Medicine, Inje University, Paik Hospital, Pusan
under 36 years old, hispanic patients (mostly Mexicans) consisted of 36.8% among these young patients. However, overall percentage of this age group with stomach cancer is 2.8% in Los Angeles and Hispanics are very high affecting.18%.
This clearly suggests that there is a strong tendency to develop stomach cancer in hispanics than other races i z the Los Angeles. My study on 350 cases of gastric cancer in Seoul, Korea, showed 10.6% belonging to this group under 36 years old.-"
Dr. S. C. Ming proposed his unique classification of gastric cancer in 1977 which was published in CANCER journal" In his paper, there is an equal occurrence of both sexes and consisted of predominant cases with infiltrating (aggressive) type of gastric cancer in young age group. In Japan, Sakamoto reported that there is a different metastatic mode of gastric cancer by age and histological type:") For undifferentiated cancer tends more to peritoneal dissemination while differentiated cancer is more prone to give rise an hepatic metastasis. And in younger patients, there are more tendencies to give rise a . peritoneal dissemination in both undifferentiated and differentiated types. Also, he noted that there are more prevalent metastasis of lymph nodes in younger patients. Koizumi and his associates examined 54 autopsy cases of non-resected gastric cancer in young and old patients, and they noted a markedly more aggressive cancer in young age group than in old age group " Kubo of Japan compared ages of stomach cancer patients between Japanese (Kyushu)
